Our providers are dedicated to staying at the forefront of medical and pharmacological advancements in the fields of disease prevention, wellness, and health improvement.

Our providers know this may not be your first attempt at losing weight, or correcting the unhealthy behaviors that brought you to where you are today. Our services and recommendations are based on extensive experience and best practices proven to give you the highest chance of success. We cannot promise it will be easy, but we can promise to provide you with every advantage possible to meet your goals.
HOW IT WORKS:
BADM has focused on leveraging technology to expand our service area. We utilize a cloud based electronic medical record in conjunction with an online payment processing service to make our services available to anyone with internet access. Features include an integrated patient portal for HIPAA compliant messaging, telemedicine visits requiring only a cell phone, and a dedicated staff to help you every step of the way.
WHAT IT INCLUDES:
1. Access to experienced staff and providers dedicated to helping you navigate your weight loss journey and achieve your goals.
2. Initial telemedicine visit with your provider to assess your individual needs and concerns. You and your provider will develop an individualized care plan that includes behavioral changes, exercise regimens, healthy food choices, and appropriate pharmacological interventions.
3. Additional telemedicine visits as needed to discuss progress, identify new barriers, and make appropriate adjustments to your care plan.
4. Access to multiple means of communication including a patient portal, direct e-mail, and a dedicated cell phone number for texting and calling. Note: Portal messaging is the preferred means of day to day communication as it is integrated into the patient chart and logs each conversation for future reference.
5. Processing of prior authorizations in order to help patient’s leverage their pharmacy benefits to offset medication costs.
6. Access to multiple compounding pharmacies, mail-order pharmacies, and patient assistance programs to provide patients with additional cost saving options.

$99 Enrollment Fee - Includes enrollment into the service, patient portal set-up, and your first telemedicine visit. You and the provider will discuss your current health status, health history, available patient resources, and develop a care plan that meets your individual needs.
$49 Monthly Fee - Recurring monthly invoice for continued access to services. Starts one month from the date of Enrollment Fee payment and recurs each month while enrolled.
Valid credit/debit card is required.
All charges are non-refundable.
Patient may cancel services at any time by phone or written request via e-mail or portal message. Upon such request, the patient’s recurring invoice will be cancelled, their chart will be made inactive, and patient will no longer be eligible for services.

STEP 1: CREATE YOUR PATIENT PORTAL ACCOUNT
To get started, please click on the click on the button below and then select the “Sign Up” link next to “Don’t have an account?”. Enter the required information and select “Continue” to complete the short 3 step process. This will create your patient portal account and one of our staff members will contact you shortly. Once logged in to your portal account, don’t forget to complete your profile by clicking on your name in the upper right hand corner and selecting the profile button.
